To provide high quality of practical support to vulnerable adults.  To support people to live as independently as possible in their own home or supported accommodation.

To support people who use the service with all aspects of maintaining the home.  These duties include: Safety and security, health and safety, use of appliances and equipment, organising repairs, paying bills, cleaning, support with meal preparation, shopping, time management, support with accessing other services.

To follow the assessments carried out by the local authority.

Park Homes UK -HFSA is an established care provider with over 20 years’ experience.

HFSA is a local provider in the Calderdale area.  We support adults with learning disabilities, physical disabilities and or mental health problems in  supported accommodation or their own homes.

Roles & Responsibilities

  • To ensure that all the people using the service are supported in accordance with their support plan/personal centered plan.
  • To know all the people you support medical conditions and to be familiar with any medication they take, and to support them with repeat prescriptions where necessary. To link it with pharmacies/ GP’s/ other medical professionals.
  • To keep accurate records of all the people you support.
  • To work as part of the team and follow the daily planners accordingly.
  • To enable people to make choices and decisions in their lives and what they do.
  • To follow the staff guidelines, policies and procedures of the company and the local authority.
  • To report any repairs to the appropriate person. Either the on call manager or landlords.
  • To carry out the Health and Safety tasks that are set where you work.
  • To report any Health and safety issues to the Health and Safety Officer or your on-call manager immediately.
  • To make sure all appointments /health issues are logged on the appropriate documentation.
  • To support people to budget, pay their bills to ensure there are no complications with their tenancy.
  • To work in accordance with Risk Assessments and to report any new identified risks to the on call manager
  • To project a professional image at all times and maintain good working relationships with all other professionals.
  • Maintain accurate records and keep up to date files ensuring, at all times, confidentiality is maintained.
  • To maintain an awareness and observation of Fire and Health & Safety Regulations
  • To carry out any other duties as are within the scope, spirit and purpose of the job, the title of the post and it’s grading as requested your manager.
  • To promote the independence of individuals supporting them in areas that have been identified only.
  • To treat people who use our service with dignity and respect at all times whilst supporting their beliefs and wishes.

Working Days

Monday to Sunday between the hours of 7am – 11pm (30hours a week)

Qualifications Required

A*-C grades in maths and english.
